The Shared Biology of Fertility
Fertility is not a woman's problem to solve alone. Research consistently shows that about one-third of infertility cases are related to female factors, one-third to male factors, and the remaining third involves both partners or remains unexplained. Conception depends on egg quality, sperm quality, timing, and the overall health of both partners.
Female Fertility by Age
In Your Twenties
Women are most fertile in their twenties. Egg quality and quantity are at their highest, and a healthy couple in their mid-20s has roughly a 20 to 25 percent chance of conceiving each month with well-timed intercourse. Pregnancy risks such as miscarriage, chromosomal abnormalities, gestational diabetes, and hypertension are lowest during this decade.
In Your Thirties
Women are born with all the eggs they will ever have — roughly 1 to 2 million at birth, declining to about 300,000 to 500,000 by puberty. Fertility begins to decline gradually around age 32 and more rapidly after 35.
By age 35, the monthly chance of conception drops to approximately 15 percent. The risk of miscarriage increases from about 10 percent in the 20s to roughly 20 percent by the mid-to-late 30s. Many women in their 30s have healthy, uncomplicated pregnancies, but additional screening is often recommended.
In Your Forties
At age 40, the natural conception rate per cycle is approximately 5 percent. By 42, it is closer to 1 to 2 percent per cycle. Miscarriage rates can climb to 30 to 40 percent or higher by the early 40s.
Risks for cesarean birth, preterm birth, stillbirth, gestational diabetes, and preeclampsia increase with maternal age. With proper monitoring and care, many women in their 40s have healthy pregnancies and babies.
Male Fertility and Age
Male fertility is part of this conversation, too. Sperm count, motility, and morphology can decline with age. Research shows that sperm quality begins to decrease gradually after age 35 and more noticeably after 40.
Advanced paternal age has been associated with a longer time to conception, higher miscarriage rates, and increased risk of certain genetic mutations. Many men in their 40s and beyond father healthy children — age is one factor among many.
Lifestyle factors have a significant impact on sperm quality. Obesity, smoking, heavy alcohol use, anabolic steroid use, poorly controlled diabetes, and exposure to toxins or high heat can all reduce sperm parameters. The good news: improving lifestyle factors can meaningfully improve sperm quality within approximately three months, which is the length of one sperm production cycle.
When to Seek a Fertility Evaluation
Clinical guidelines recommend the following:
•If you are under 35 and have not conceived after 12 months of trying, seek evaluation.
•If you are 35 or older and have not conceived after 6 months of trying, seek evaluation.
•If you are 40 or older, consider earlier consultation.
Ovarian reserve testing, such as AMH levels and antral follicle count, can provide more personalized information about your fertility picture.
